We offer exceptional home care services, with personalized programs that are ideal for you or a family member. By developing an individualized care plan, we can fulfill each client’s requirements and scheduling preferences by creating a care program that is tailor-made to the individual.

Hospice & Palliative Care

Pain management, spiritual care and hospitalization prevention are all components of the Hospice & Palliative Care program. Our compassionate care provides comfort for those who need chronic care and end-of-life life support, facilitating not only the patient, but also the individual’s support network. Since every person’s path is different, the services we offer are tailored to meet the patient’s specific needs. Palliative and hospice care can be given in long-term care facilities, assisted living facilities, and at home.

What is the difference between palliative care and hospice care?

While both palliative care and hospice care offer comfort, palliative care can begin with a diagnosis and, if warranted, treatment. Hospice care begins after treatment of the disease is stopped and all options have been explored. Hospice care and palliative care address an individual’s physical, emotional, and spiritual needs. Hospice care can help with such daily activities as administering medications, bathing, and dressing.

What does the hospice admission process involve?

One of the first things the hospice program will do is contact the person’s physician to make sure he or she agrees that hospice care is appropriate for the patient. The client will be asked to sign consent and insurance forms. These are similar to the forms patients sign when they enter a hospital.

Why do individuals and families choose hospice care?

For seriously ill individuals, hospice and palliative care is a way to receive the most appropriate and compassionate care. It can help improve the quality of life, bringing a measure of comfort to all involved.
